What is wrong with this girl's face?
Diagnosis: Irritant dermatitis with secondary excoriation and impetigo
She told me that she started an acne treatment program with a benzoyl peroxide containing preparation that her grandmother purchased after seeing on television. She started using it about a month ago. Since starting the product she reports that her face has become very itchy and progressively worse.
I suspected that the OTC acne product provoked an irritant dermatitis that initiated Acne Excoriée (picking behavior) that caused (mild) secondary impetigo.
Treatment approach: I told her to stop the TV acne treatment, use Alclometasone dipropionate cream BID x 7 days and Keflex 500 BID x 7 days. This was her on day 11 after starting that program! The patient and her family were very happy with the results..
