A 9-year-old girl presents with bumps around her mouth of 4 weeks duration. Her mother said she is getting progressively worse and they are applying moisturizing lotion once or twice daily without benefit. She is otherwise healthy.

Diagnosis: Childhood granulomatous periorificial dermatitis
Childhood granulomatous periorificial dermatitis (CGPD) is an uncommon granulomatous variant of perioral dermatitis. I treated this (70lb) little girl with topical Metrocream, Elidel cream (each QD) and oral Zithromax (1/2 tab daily x 1 month). She came back at day 30 and was still active, with about 85% improvement. As a result, I extended her treatment for 2 more weeks. These photos represent baseline and follow up at week 6. Thanks for everyone's input on this case.
