Case of the Month

A 73-year-old man (physician) presented with a small cluster of very pruritic vesicles on his right clavicular area. He reported that he had been working in his wooded yard and garden over the weekend. I made the initial diagnosis of allergic contact dermatitis. He came back to clinic 4 days later after developing this very large red, round patch.
What's Your Diagnosis?
Diagnosis: Lyme Disease
Testing revealed a positive Lyme’s test. He was started on Doxycycline, 100mg, BID for 21 days. He called 7 days later to report 90% resolution.
