Diagnosis: Gianotti-Crosti Syndrome

Gianotti-Crosti Syndrome, also known as 'papular acrodermatitis of childhood' is an infectious exanthem that results mostly from a wide variety of viral infections. The rash is also seen, to a much lesser degree, after bacterial infections and vaccinations. The initial reports by Gianotti and Crosti were of European children infected with hepatitis B virus. A distinction was made between the hepatitis B form and the non-hepatitis B associated versions, but research has failed to elucidate a significant difference between them, so they are both now considered to be the same disease. The most common cause of Gianotti-Crosti in the United States is Epstein Barr virus.

The exanthem will usually last 4-8 weeks, but contrary to some thinking, in symptomatic patients I have used mild topical steroids and narrow-band UVB phototherapy to decrease the exanthem duration to 10-14 days. The rash, however, will almost always resolve without intervention in 4-8 weeks.
