Diagnosis: Lentigo Maligna
There has been much confusion over the terminology and exact meaning of 'lentigo maligna' (see eMedicine CME article below).
I would like to simplify the discussion by agreeing completely with A. Bernard Ackerman, M.D.:
"Lentigo maligna is melanoma-in-situ on a background of sun damaged skin".
Treatment includes surgical excision of this superficial melanoma. More and more frequently, Mohs micrographically margin controlled surgery is the excisional approach. In patients in declining or poor health, radiation may be a reasonable consideration. For melanoma-in-situ, the appropriate medical tests, in addition to a general medical exam, include liver function studies and a chest x-ray. Patient should be followed, clinically, at approximate 3-6 month intervals.
